v2.0: Improved question quality based on these principles:
- Questions must yield actionable answers
- Answers should be facts, not opinions
- No excuse-enabling questions
- Closed questions with qualifiers, not open-ended
- Literal language, no jargon or buzzwords
- Flip to opposite framing when more useful
- Complex topics become procedures, not single questions
- No emotional/subjective framing
- No ego-serving questions
- Every question tested against: "Does this help figure out how to get there,
  what to do once there, and how conditions change?"
